Home care provided by Blue Ram is implemented through our Care Plan Program, collectively involving the family of our clients, Blue Ram caregivers and Blue Ram care directors. Our care plans are designed to encourage clients' independence; and we create plans anchored around a healthy diet, exercise and social engagement.

Our team has also developed a Dementia Home Care Program - catered specifically for clients experiencing the complications of memory loss and its detriment to living independently.

We will schedule a meeting in the client's home for a consultation with the client, family members and Blue Ram care directors - one of which is a registered nurse. During this time, we listen attentively to all of the care needs from the client and family members. Additionally, this time together provides the opportunity to build rapport and learn more about one another.

During the consultation, our care director RN will review all of the client's available medical records and perform a mental & physical evaluation. The client's level of mobility, degree of independence, dietary requirements and medication regiment are all examples of what is covered during the assessment.

Our directors will also assess the client's home for potential barriers to providing care or safety concerns within the home. Assistive devices (such as a wheelchair or walker) and adaptive devices (such as installing bars to safely get in and out of the shower) are all considered during this process.

After our in-home consultation, our team members will review all of the client's needs and assign the perfect Blue Ram caregiver. Weekly scheduling will be confirmed and all components set in motion to begin the process of providing the best care imagineable.

One of the most important features of Blue Ram Home Care is our continuous evaluation of our clients' satisfaction and needs. We achieve this with a high degree of communication and documentation between all team members. Our caregivers observe any changes in physical abilities, cognition and discover each clients' likes and dislikes in great detail. All caregiver observations and suggestions are relayed to the Blue Ram care directors for fine-tuning of our services. Care directors also make frequent visits to our clients' homes, fostering our mission of providing care from an entire team of loving and compassionate individuals.
