We are seeking a dedicated Occupational Therapist (OT) to join our home health care team and make a meaningful impact in the lives of our patients.

Position Summary:

The Occupational Therapist will provide in-home therapy services to patients who require assistance in regaining independence in daily living activities. This role focuses on evaluating patient needs, creating individualized treatment plans, implementing therapeutic activities, and educating patients and families on adaptive strategies to improve safety, mobility, and overall independence.

Key Responsibilities:

  • Conduct initial assessments of patients’ functional abilities, home environment, and daily living needs.
  • Develop and implement individualized therapy plans in accordance with physician orders.
  • Provide therapeutic interventions to improve mobility, strength, coordination, and independence in activities of daily living (ADLs).
  • Educate patients, caregivers, and families on safe techniques, adaptive equipment, and home modifications.
  • Document evaluations, progress notes, and treatment updates in compliance with agency, state, and federal guidelines.
  • Collaborate with physicians, nurses, physical therapists, and other healthcare providers to ensure coordinated care.
  • Adjust therapy goals and plans based on patient progress and evolving needs.

Qualifications:

  • Current and active Occupational Therapist (OT) license with the North Carolina Board of Occupational Therapy.
  • Bachelor’s, Master’s, or Doctorate in Occupational Therapy from an accredited program.
  • Prior experience in home health, rehabilitation, or geriatric care preferred.
  • Strong communication, problem-solving, and organizational skills.
  • Ability to work independently and manage a flexible caseload.
  • CPR certification (required).
  • Valid driver’s license and reliable transportation.

Blue Ram No Smoking Policy

(a) Home care agencies and home assistance service providers shall prohibit their employees from smoking while providing services to an individual in the individual's home. Home care agencies and home assistance services providers shall inform their clients that employees are prohibited from smoking in a client's home. As used in this section:

      (1.) "Employee" includes an individual under contract with the home care agency to provide home care services and an individual under contract with a home assistance services provider to provide home assistance services.

      (2.) "Smoking" means the use or possession of any lighted cigar, cigarette, pipe or other lighted smoking product - inluding vaporized smoking products.

(b) The Department may impose an administrative penalty not to exceed two hundred dollars ($200) for each violation on any person who owns, manages, operates or otherwise controls the home care agency or home assistance services provider and fails to comply with the section. A violation of this section constitutes a civil offense only and is not a crime.
